Lloyds Enable: Settled Futures Project

The Lloyds Bank Foundation has awarded DART (Disability Advice Resource Team) a grant to run a small pilot project. From June 2017 they will provide a holistic support service for people over the age of 65 who are facing upheaval in their lives due to deteriorating health and/or changes of circumstances which threaten their independence. The Project will reach out to those who are isolated and have health problems. DART will work with health and social care providers to identify those recently hospitalised or those with a self-identified crisis and concerns about their future.

They will Work with clients face to face over several sessions to develop trust and confidence so that their concerns and issues can be successfully identified and addressed. The clients can then be supported to access appropriate services e.g. housing aids adaptations, access to social activities in order to help them maintain independence and dignity.
